POSITION TITLE

Senior & Disability Services Resource Specialist - Exempt

INTERNAL RELATIONS:

Reports to the Director of Senior & Disability Services

HOURS:

Position is full-time, 32.5 hours/week, 9:00am - 4:30pm

POSITION OVERVIEW:

  • Serve as SHIP Counselor (Senior Health Insurance Program);
    • Provide Medicare counseling and assistance;
    • Provide Benefit Access assistance;
  • Provide redetermination assistance for Benefit Access, Medicaid, etc;
  • Provide intake and redetermination assistance for RTA;
  • Provide information, referrals and assistance pertaining to senior and disabled residents in Wheeling Township and/or and their families;
  • Offer community presentations/meet with community groups/attend community fairs/represent Wheeling Township in a marketing/outreach capacity, as needed;
  • Develop and maintains a network of individuals and organizations affiliated with Wheeling Township senior citizens and residents with disabilities;
  • Serve as Home Delivered Meal Program backup;

JOB REQUIREMENTS

  • Knowledge of - and sensitivity to -- working with older adults and the permanently disabled who may have limitations of hearing, eyesight, mobility or cognitive impairment;
  • Excellent written communication and public speaking/presentation skills;
  • Excellent organization skills;
  • Outstanding attention to detail;
  • Excellent listening skills and compassion for resident/family;
  • Capacity to efficiently, effectively and professionally interface with Township staff, residents and the general public;
  • Ability to drive an automobile and deliver meals to a resident's door;
  • Ability to lift 15lbs;
  • Availability to work occasional nights/weekends/holidays, as required;
  • Others as specified

POSITION DETAILS:

SENIOR SERVICES

  1. Determine resident's/family's needs through intake and investigation; provide information on external and internal services; provide external/internal referrals and advocacy as needed;
  2. Provide Medicare assistance as a certified SHIP Counselor, with the goal of becoming SHIP Site Coordinator; provide Benefit Access assistance;

*Attend all relevant Medicare/SHIP/Age Options trainings as necessary;

  1. Provide Medicaid, Benefit Access and RTA redeterminations, advocacy and engagement, as needed;

*Attend relevant Medicaid/Benefit Access/RTA/Age Options trainings as necessary;

  1. Provide assistance to residents requiring help with ITAC and other government services;
  2. Provide community engagement presentations, including those to local senior facilities, churches, civic groups, etc.;
  3. Learn about Home Delivered Meal Program and provide backup assistance in scheduling, planning, meal delivery and volunteer engagement, when needed;
  4. Others, as specified

OTHER

  • Assist the Director of Senior & Disability Services with the following:

*Volunteer Programs/Communication;

*Support Groups;

  • Attend appropriate training/workshops on topics related to senior and/or disability services;
  • Write and submit articles for the Township quarterly Newsletter, meeting established publication deadlines.

QUALIFICATIONS & EXPERIENCE

  • Minimum of 3+ years' experience in senior services or related field;
  • College degree required, ideally in relevant discipline;
  • Successfully complete SHIP training within 6 months (if not already certified);
  • Successfully complete CPR training;
  • Must pass criminal record background check, including National Sex Offender and fingerprint;
  • Must possess good driving record (MVR);
  • Negative pre-employment drug check required;
  • Availability to work occasional nights/weekends/holidays, as required;
  • Reliable vehicle for community outreach and service projects; valid IL Driver's License and proof of auto insurance;
  • Knowledge of service area and/or capacity to learn quickly;
  • Computer skills, including Microsoft Office applications.
